Senior Instructor Signs (if applicable)
If you are applying as Sempai, your Sensei (Senior Instructor) must fill the recommendation section and sign.

Chief Instructor Signs
Your Renshi (Chief Instructor) endorses the application and signs. For Renshi-level applications, the Director signs.

Submit to your Director / HQ
The completed, signed form is submitted to your Hanshi (Director) or directly to HQ for final review.

Receive your Instructor ID
Once approved, HQ will issue your Instructor ID and portal login credentials by email.
